A DOT physical exam is the evaluation required in order to operate a commercial vehicle. It is referred to as a “medical fitness for duty” examination, which ensures that each driver is capable and healthy enough to successfully complete the duties associated with commercial driving. The health standards are determined by the FMCSA and each DOT physical exam must be conducted by a certified medical professional to ensure the high quality standards are met during each exam.

  • Have two forms of Identification when applying at a Highway Safety and Motor Vehicle office (appointments are recommended).
  • Have available your social security number.
  • Proof of your residential address.
  • Must be at least 18 years of age.
  • Meet medical eligibility requirements and show a valid Medical Examination Report.
  • Pass all of the required tests.
  • Pay the applicable fees.

Health Requirements

Your blood pressure must meet the following criteria:

1. Less than 140/90 for 2 year certificate

2. 140-159/90-99 for 1 year certificate

3. 160-179/100-109 for a 3 month temporary certificate

4. Greater than 180/110 is disqualifying

* Prescription medication to control blood pressure is permitted.

  • Use of a Schedule 1 drug or any other habit forming drug is not permitted.

Your examination results need to be recorded on a DOT Physical Form by a licensed medical examiner such as a Doctor of Chiropractic. Our chiropractic office has these forms. However, just to be sure, you can download your DOT Medical form from various sources online (including the federal government) and take it with you to the doctor’s office.

You must pass the DOT physical in order to qualify for a CDL as your health plays an important factor in driving safety, especially considering the vehicles you’ll be driving.

  • You must have 20/40 correctable vision in each eye. Glasses or contact lenses are acceptable to correct a driver’s vision.
  • You cannot be a diabetic on needle-injected meds; diabetes controlled through a diet or oral medication is permitted.
  • If you have a current diagnosis of cardiac insufficiency, collapse, congestive cardiac failure or any other cardiovascular disease you will be required to provide the Medical Examiner with a stress test (needs to be performed within the last 12 months) along with a release from your physician stating that you can drive a commercial motor vehicle without restrictions
  • Other factors or conditions which could prevent you from obtaining the required DOT clearance include: sleep apnea, recent back injury, recent major surgery, a current hernia, or recent workmen’s compensation claims.
